Fees, Insurance Coverage, and Payment Policy
We want you to be completely comfortable with the financial arrangements for your surgery. If you have a question that isn’t answered here, please call us at (208) 453-8668.
Our facility fees cover expenses like routine lab work, medication, the operating and recovery room, and all supplies. It does not include laboratory, pathology, surgeon, anesthesiologist, or certified nurse anesthetist fees. You will be billed separately for these fees.
We accept cash, cashier's checks, VISA, MasterCard, Discover Card, American Express, and personal checks with a valid I.D. If you require special financial arrangements, please phone our office prior to your surgery to discuss alternative methods of payment.
Some procedures like cosmetic surgery and dental work are often not covered by insurance. You will be asked to pay these charges in full at the time of surgery.
Remember, because we are a cost-effective alternative to hospitalization, you can expect our charges to be fair and reasonable.
If you have Insurance
We will verify your eligibility and coverage with your insurance company and bill your primary and secondary insurance carriers or governmental agency directly for the facility's charges. If you do not have 100% coverage you will be required to make a payment on the day of your surgery.
Be sure to bring your most current insurance, Medicare, or public assistance card with you on the day of your surgery. If you have more than one insurance carrier, we will also need accurate secondary billing information. Please be prepared to pay any co-pay or deductible amounts on the day of your surgery.
Please be aware of any admission policies your insurance plan may have. You or your physician may have to adhere to certain requirements in order to insure maximum reimbursement. Failure to obtain pre-authorization, physician referral, or a second opinion may greatly reduce or eliminate your benefits.
If you have Medicare
We welcome Medicare beneficiaries at this facility. We are certified by Medicare and comply with specified physical plant, staffing, safety and governance standards.
Medical covers facility fees for approved services at an Ambulatory Surgery Center. However, you are responsible for co-insurance and Part B deductible applies. For more information on Medicare and your benefits, please visit the Medicare website.
If you do not have Insurance
Patients who do not have insurance coverage or those having cosmetic surgery are also required to pay their charges in advance. If you are having cosmetic surgery, no personal checks will be accepted.
